How to Get Support Coordination in Your Plan
Demonstrate your need
Gather evidence of why you need help implementing your plan — a letter from your GP or treating specialist, documentation of complex needs, or a history of difficulty navigating the NDIS.
Request it at your planning meeting or review
Tell your NDIA planner or LAC that you want Support Coordination included in your plan. Explain what you need help with — connecting with providers, managing complex supports, or preparing for future reviews.
Choose your support coordinator

Sign a service agreement and get started
