In the 12 months to 30 June 2025, the NDIS delivered $45.9 billion in support to over 739,000 Australians living with disability. The scheme now operates through more than 269,000 registered and active providers, making it one of the largest social care markets in the world.

Despite this scale, the provider market is highly fragmented. No single organisation holds more than approximately 1.3% of total scheme payments, and the top 25 providers combined account for roughly 10% of national NDIS expenditure. This list reflects the organisations that are consistently named among the largest by revenue, participant numbers, and geographic reach as of 2025.

1. Life Without Barriers

Life Without Barriers is one of Australia's largest NDIS providers by total payments received, operating nationally across all states and territories. The organisation provides supported independent living, disability employment, out-of-home care, and mental health support. Its breadth of service and national workforce make it the closest thing the sector has to a dominant player, though its market share remains below 1.3%.

2. Aruma

Aruma was formed through the merger of House with No Steps and The Tipping Foundation and operates across New South Wales, Queensland, Victoria, and the ACT. It delivers accommodation, employment assistance, therapy, and community participation services. Aruma's scale in supported independent living and its geographic spread across multiple high-population states place it consistently among the top-tier providers by revenue.

3. Scope Australia

Scope Australia is a leading provider of therapy and communication support for people with physical and intellectual disabilities, primarily in Victoria and New South Wales. It is particularly known for its work in augmentative and alternative communication (AAC) and assistive technology. In the 2024-25 period, Scope continued to expand its allied health workforce to meet growing demand for therapy services.

4. Cerebral Palsy Alliance

Cerebral Palsy Alliance (CPA) is Australia's largest provider of services to people with cerebral palsy, operating primarily in New South Wales. Its services include therapy, assistive technology, research, and advocacy. CPA's research investment and early intervention programs are internationally recognised, and its specialised model continues to drive strong referral volumes from paediatricians and hospitals.

5. Endeavour Foundation

Established in 1951, Endeavour Foundation is one of Queensland's largest and most established disability organisations. It provides supported employment, accommodation, life skills training, and community programs. Endeavour Foundation has been navigating the ongoing transition of Australian Disability Enterprises toward more inclusive employment models, a shift that affects its supported employment revenue structure.

6. Northcott

Northcott is one of the largest providers in New South Wales, Queensland, and the ACT, supporting people with a wide range of disabilities. Services include behaviour support, early childhood intervention, home modifications, and supported independent living. Northcott's strong presence in regional NSW and its investment in digital delivery have helped it extend reach beyond major metropolitan centres.

7. Yooralla

Yooralla is one of Victoria's largest and longest-standing disability providers, with over 100 years of history in the state. Services include day programs, respite, supported residential services, and employment support across metropolitan Melbourne and regional Victoria. Like many large Victorian providers, Yooralla has undertaken service line restructuring in response to cost pressures in the current pricing environment.

8. Achieve Australia

Achieve Australia has delivered disability services for over 65 years across New South Wales, focusing on individualised support plans that link daily supports to participant goals. Services include supported accommodation, community participation, and employment. Achieve is known for its co-designed approach to support planning, which has translated into strong participant retention.

9. CPL — Choice, Passion, Life

CPL is one of Queensland's largest NDIS providers, with a strong focus on allied health therapies, early childhood intervention, and assistive technology. With autism now the primary disability for 43% of all NDIS participants nationally, CPL's early childhood and autism-specific therapy services are in high demand across southeast Queensland.

10. genU

genU (formerly Karingal St Laurence) operates across Victoria, New South Wales, and South Australia with a focus on employment, training, and community participation. Its registered training organisation and social enterprise model provide pathways for participants seeking vocational outcomes. GenU continues to be a significant provider in the employment and day activities space in the southern states.

11. Sunnyfield

Sunnyfield has supported people with intellectual disabilities in New South Wales since 1952. Services span employment, care at home, community participation, and supported accommodation. Sunnyfield's metropolitan and regional NSW footprint and its established workforce in supported employment give it consistent participant volumes across the state.

12. Ability Options

Ability Options operates across Greater Sydney and regional NSW, specialising in housing, employment, and community support. It is one of the larger mid-tier providers in the state and has a strong focus on creating long-term independence pathways through supported living and open employment placements.

13. Activ Foundation

Activ is one of Western Australia's largest disability organisations, operating employment, lifestyle support, and community programs across metropolitan Perth and regional WA. Activ's social enterprise operations provide both commercial revenue and employment outcomes for participants. The organisation restructured several supported employment operations during 2024 in response to national pricing changes.

14. Rocky Bay

Rocky Bay is a leading Western Australian provider with expertise in assistive technology, rehabilitation, vocational training, and community programs. It operates across metropolitan Perth and regional WA, with a strong therapy and equipment funding capability. Rocky Bay has grown its allied health capacity to address high demand in Perth's expanding northern suburbs.

15. Therapy Focus

Therapy Focus is Western Australia's largest community-based therapy provider, supporting children and adults with disability through multidisciplinary teams. Occupational therapists, physiotherapists, speech pathologists, and psychologists deliver services in homes, schools, and community settings across the state. Its capacity in early intervention has grown significantly following the NDIS early childhood approach changes.

16. Autism Spectrum Australia (Aspect)

Aspect is Australia's largest autism-specific service provider, operating specialist schools, early intervention, adult support, and consultancy services. With autism now representing 43% of all NDIS participants — a figure that has grown significantly over recent years — Aspect's specialised expertise and evidence-based programs place it in a uniquely strong market position.

17. Novita

Novita is South Australia's leading disability service provider, specialising in paediatric disability and early intervention. Services include occupational therapy, physiotherapy, speech pathology, and assistive technology. Novita has invested significantly in early childhood services following the NDIS early childhood approach reforms introduced in recent years.

18. Afford

Afford (The Afford Group) operates a wide network of activity centres and support services across Sydney and regional New South Wales. Its services include community participation, supported independent living, and short-term accommodation. Afford has positioned itself on accessible pricing and convenience, with strong referral volumes from school-leaver transition programs and secondary schools.

19. Uniting (NSW/ACT)

Uniting is the community services arm of the Uniting Church in NSW and the ACT, operating one of Australia's broadest social service networks across disability, aged care, and family services. Its disability division delivers NDIS supports including in-home care, community access, and supported accommodation across a large geographic footprint in NSW.

20. Mosaic Community Care

Mosaic Community Care provides integrated support across disability, mental health, and aged care in Western Australia. Its model is particularly relevant for participants with dual diagnoses, a cohort that presents complex challenges for mainstream providers. Mosaic's Perth-based operations include community participation, in-home support, and short-term accommodation.

21. Intereach

Intereach operates primarily in rural and regional New South Wales and Victoria, addressing the persistent gap in NDIS service availability outside metropolitan areas. Its focus on regional and remote communities makes it one of the more important providers for participants in areas where mainstream providers have limited or no presence.

22. Access Community Services

Access Community Services delivers NDIS supports, settlement services, and community development programs in Queensland, with a strong focus on culturally and linguistically diverse communities. As the NDIS increasingly works to engage participants from non-English-speaking backgrounds, providers like Access with bilingual workforces and cultural competency frameworks have become increasingly relevant.

23. St Vincent de Paul Society

The St Vincent de Paul Society operates disability support services across multiple states as part of its broader charitable mission. Its NDIS services focus on community participation, social connection, and in-home support for participants with lower-intensity needs. Its volunteer and community infrastructure supplements its paid disability workforce.

24. Cara

Cara is South Australia's second-largest disability service organisation, delivering accommodation, community support, respite, and therapy services across Adelaide and regional South Australia. Its long presence in the SA market and established accommodation portfolio have maintained its position among the state's key providers through the scheme's ongoing evolution.

25. The Benevolent Society

The Benevolent Society is Australia's oldest charity, having operated social services since 1813. In September 2024, it made the significant decision to exit support coordination services across Queensland, New South Wales, Victoria, and the ACT, citing the unsustainable financial model under current NDIS pricing. The exit is notable as a signal of the pressures facing large generalist providers who cannot achieve efficiency at scale under the current rate structure.

Key trends shaping the sector in 2025

Three trends define the NDIS provider landscape in 2025. First, large providers are reporting significant operating losses — nine of the ten largest providers by revenue recorded negative adjusted operating results, with an average loss of 5.8%. Second, the provider base is becoming more specialised, with generalist organisations exiting low-margin services. Third, autism and early childhood intervention services are the fastest-growing demand areas as the participant demographic evolves.

For NDIS participants across Perth, the national picture highlights the importance of choosing providers with genuine local capacity rather than relying on large organisations whose attention is spread across multiple states and service lines.
